High cervical stimulation refers to the surgical placement of a spinal cord stimulator device at the upper level of the cervical spine, close to the base of the skull, to help manage certain chronic pain conditions affecting the head, neck, and upper limbs that have not responded to more conservative treatment. The procedure involves placing thin electrode leads in the epidural space near the spinal cord at this high cervical level, connected to a small implanted pulse generator that delivers mild electrical impulses to interfere with pain signal transmission before it reaches the brain. It is generally considered for conditions such as occipital neuralgia, certain forms of chronic headache, and complex regional or neuropathic pain syndromes involving the upper body, after other treatments have failed to give adequate relief. Most patients undergo a temporary trial with external leads first, so that both patient and physician can assess how much pain relief the stimulation provides before committing to a permanent implant. If the trial is successful, the permanent system is implanted in a subsequent procedure. India's pain management centres offer both the trial and permanent stimulation stages along with the follow-up programming this therapy requires.
