IVF with TESA combines standard in vitro fertilization with testicular sperm aspiration, a minor procedure used to retrieve sperm directly from the testicle when none is present in the ejaculate, a condition known as azoospermia. Under local anesthesia, a fine needle is inserted into the testicle to withdraw a small sample of tissue or fluid containing sperm, which is then examined in the laboratory to identify viable sperm cells for use in fertilization. Because sperm retrieved this way is often limited in number, it is typically combined with ICSI, where a single sperm is injected directly into each mature egg, rather than conventional insemination in a dish. On the female partner's side, the process follows the usual IVF path of ovarian stimulation, monitoring, and egg retrieval, timed to coincide with the TESA procedure so that fresh sperm and eggs are available together, though sperm can also be frozen in advance. This approach is used for men with obstructive or non-obstructive azoospermia where sperm production continues in the testicle despite an absent or blocked path to ejaculation. Patients traveling to India for IVF with TESA typically plan their visit around the combined timeline of ovarian stimulation and the sperm retrieval procedure.
