About Azoospermia Treatment

Azoospermia is the complete absence of sperm in the ejaculate, and its treatment depends on whether the cause is obstructive, meaning sperm is produced normally but blocked from reaching the ejaculate, or non-obstructive, meaning sperm production itself is impaired. Diagnosis involves hormone blood tests, semen analysis, scrotal ultrasound, and sometimes genetic testing to distinguish between the two types and identify the underlying cause, such as a prior vasectomy, congenital absence of the vas deferens, infection, or a hormonal or testicular disorder. Obstructive cases may be treated by surgically reconnecting or bypassing the blockage, such as vasectomy reversal, or by retrieving sperm directly from the testicle or epididymis using minor procedures like TESA, PESA, or micro-TESE. Non-obstructive cases are managed first by correcting any underlying hormonal issue where possible, and if sperm is still not present in the ejaculate, surgical sperm retrieval may still locate small pockets of sperm production within the testicle for use with ICSI. Retrieved sperm, when found, is typically used immediately or frozen for a subsequent IVF/ICSI cycle. Patients traveling to India for azoospermia treatment usually undergo diagnostic testing early in the visit to determine which retrieval approach is appropriate before proceeding.
