About Biliopancreatic Diversion

Biliopancreatic diversion, often performed with a duodenal switch, is one of the more complex and powerful bariatric procedures, combining a sleeve gastrectomy with a significant rerouting of the small intestine to substantially limit calorie and nutrient absorption. The surgeon first creates a sleeve-shaped stomach similar to a standard sleeve gastrectomy, then divides the small intestine so that food travels through only a short common channel where it mixes with bile and pancreatic digestive juices, meaning most of the small intestine's absorptive length is bypassed for calories and fat in particular. This produces greater and more durable weight loss than sleeve gastrectomy or gastric bypass alone and can markedly improve type 2 diabetes, but it also carries a higher risk of nutrient and vitamin deficiencies, requiring strict lifelong supplementation and monitoring. It is generally reserved for patients with very high body mass index or severe obesity-related metabolic disease who have not responded adequately to other approaches. Performed laparoscopically under general anesthesia, the operation typically takes three to four hours given its technical complexity. Indian bariatric centers offering this procedure usually reserve it for carefully selected patients after thorough metabolic and nutritional evaluation, with close long-term follow-up to manage its more demanding nutritional requirements.
