Corneal cross-linking is a procedure designed to strengthen the cornea's internal collagen structure and halt the progression of keratoconus and other conditions that cause the cornea to gradually thin and bulge into an irregular, cone-like shape. During the standard, or Epi-off, technique, the thin outer layer of the cornea is gently removed, riboflavin, or vitamin B2, eye drops are applied to saturate the corneal tissue, and the eye is then exposed to a controlled dose of ultraviolet-A light, which triggers a chemical reaction that creates new molecular bonds between collagen fibers, increasing the cornea's rigidity. A newer variation, Epi-on cross-linking, leaves the outer layer intact, which can shorten recovery time though it may be less effective in certain cases. The procedure is typically recommended once progressive thinning has been confirmed through repeated corneal mapping, most often in younger patients whose keratoconus is still advancing, with the goal of preventing further deterioration rather than reversing existing damage. Recovery from the standard technique generally takes several days to a week. Eye centers in India commonly combine cross-linking with regular topography monitoring to track corneal stability before and after treatment.
